Kindergarten Registration

2012-2013 School Year

WHO:  Children who are residents of North Smithfield and who are/will be

5 years old on/before September 1, 2012.

WHEN: Tuesday, March 27, 2011 (5:00 pm – 8:00 pm)

AND

Saturday, April 28, 2011 (9:00 am – 12:00 noon)

WHERE: North Smithfield Elementary School

2214 Providence Pike

WHAT PARENTS/GUARDIANS NEED TO BRING:

1.)     Birth Certificate or Passport

2.)     Proof of Residency (current utility bill, lease agreement)

3.)     Immunizations/Physical Record:  records MUST have the date

of each immunization in accordance to RIDOH for 5 year olds as

well as proof of vision and lead screening along with  a completed physical exam

4.) Photo Identification
